AI Guides World-First Brain Surgery, Saves Sight

Surgeons in London removed a brain tumour from a 48-year-old man while an artificial intelligence system read the live camera feed and told them where not to cut — the world's first AI-assisted brain surgery on a patient, doctors said Thursday.
The patient, Rhys Hibbert, a customer services manager and father of two from Bedfordshire, was facing blindness. An 11mm non-cancerous tumour on his pituitary gland was pressing on the optic nerves.
The operation was performed in May at the National Hospital for Neurology and Neurosurgery, part of University College London Hospitals, and kept under wraps until Thursday while Hibbert recovered, The Guardian reported.
How It Worked
Surgeons fed an endoscope — a thin camera on a stick — through Hibbert's nose to the base of his skull, where the tumour sat hidden behind bone and membrane.
On a second screen, the AI analysed the live video, tracked the surgical instruments and colour-coded the vessels and nerves most likely to be in the way, marking the safest places to cut. The BBC compared it to the facial recognition on a phone, except it recognises hidden anatomy.
The pituitary gland sits beside the carotid arteries and the optic nerves. Health officials said going a millimetre wrong can mean blindness, stroke or death. According to the BBC, the procedure carries a 25 to 50 percent chance of not getting the whole tumour out and a 0.5 to 2 percent chance of injuring a major blood vessel.
The system was trained on hundreds of videos of pituitary tumour removals, with the vessels and nerves drawn in by hand on each one. The average UK surgeon performs about 10 to 20 of these operations a year.
Timeline
Hibbert collapses and starts fitting in the road during one of his daily two-mile lunchtime walks. A scan finds the 11mm pituitary tumour. He is managed without surgery at first.
Symptoms worsen: narrowing peripheral vision, severe hormone imbalance, fatigue, dizziness, tripping over things. Without surgery, doctors said, he faced going blind.
Prof Hani Marcus and Mr Danyal Khan operate at the National Hospital for Neurology and Neurosurgery using the AI tool for the first time on a patient. Within a week Hibbert is walking independently without glasses or sticks.
UCLH and health officials make the operation public.
Reactions
"When I opened my eyes after the operation it felt like I had got 360-degree vision — I hadn't seen that clearly for a year," Hibbert told the BBC. "It's given me my life back."
He said he did not hesitate when offered the chance to go first: "If patients are not prepared to join research how can doctors ever learn and how can medicine ever progress?" On the safety case, he added: "There are no road signs inside our head."
Marcus said the tool "is trained on more operations than most surgeons see or do in a lifetime and can act like an expert second pair of eyes," and stressed that the surgeons kept full control. His long-term goal, he said, is a kind of ChatGPT for surgeons — "another expert in the room they can turn to for advice if they want it — or leave if they disagree with it."
Dr Sophia Bano, an associate professor in robotics and AI at UCL and the technical lead on the system, said it "is designed to help recognise critical anatomy, surgical instruments and tissue interactions in real time, supporting the surgeon during highly delicate procedures."
James Frith, the UK's health innovation minister, said he was pleased Hibbert had "life-changing surgery" through government-funded research, adding: "AI needs proper safeguards and we will always ensure that safety is taken seriously. But we will also ensure we benefit from the opportunities it brings."
What's Next
The team is working on a larger trial after years of lab testing. The research was funded by the National Institute for Health and Care Research and by Google.
Prof Mike Lewis, NIHR's scientific director for innovation, said the "pioneering surgery" showed the potential of advanced AI to support surgeons and improve patient care.
Hibbert has returned to work. He said his sight has kept improving in the weeks since the operation and the side effects he had before surgery are gone.

Hibbert noted the setting: the National Hospital for Neurology and Neurosurgery was founded in 1859 and was the world's first dedicated neurosurgical hospital. "So to me, it seems very fitting that the same hospital should also be the world's first to carry out an AI-assisted neurosurgery," he said.
