Scientists Find New Virus in Tick Invading US

Scientists in China have identified a new tick-borne virus that causes a flu-like illness — and the tick that carries it is already established across more than two dozen US states.

The researchers, led by the State Key Laboratory of Pathogen and Biosecurity in Beijing, named it the Asian longhorned tick nairovirus, or ALTNV. Their findings were published in the New England Journal of Medicine and reported by CIDRAP, the University of Minnesota's Center for Infectious Disease Research and Policy.

The Asian longhorned tick was first discovered in the United States in 2017 and has since spread to more than two dozen states, according to The Hill and Nexstar stations covering the study.

Every patient and every tick in the study was in China. The researchers did not report any US cases.

The Numbers

Of 3,163 patients tested, 10.4% were positive for ALTNV by RNA or IgM antibody detection. Among patients who tested negative for Dabie bandavirus — the tick-borne hemorrhagic fever the team was originally chasing — 15.1% were positive for ALTNV.

ALTNV RNA turned up in 1.3% of 47,428 ticks sampled across 15 Chinese provinces, with the highest rate, 1.8%, in Haemaphysalis longicornis — the Asian longhorned tick.

Reactions

The study authors framed the risk as a diagnostic problem first. "The cocirculation of ALTNV and DBV in the same areas, coupled with their shared tick vector, underscores the need for improved differential diagnosis between infections with these viruses in regions where SFTS is endemic," they wrote, referring to severe fever with thrombocytopenia syndrome, the disease DBV causes.

CIDRAP reported the illness as an influenza-like syndrome marked by fever, fatigue, gastrointestinal symptoms and abnormal lab findings.

The US coverage that put the story in front of American readers led with the tick's spread rather than with alarm, noting the species has moved through more than two dozen states in under a decade.

What's Next

The researchers called for better differential testing in areas where SFTS is endemic, so patients infected with ALTNV are not misread as DBV-negative mysteries.

The open question for the United States is whether ALTNV travels with the tick. The virus has not been reported here, and the study did not test American ticks or patients.

Patients infected with ALTNV alone most often had fatigue (84%), gastrointestinal problems (80%), low platelet counts (38%) and elevated aminotransferase levels indicating liver or tissue injury (36%). Those patients recovered completely.

Coinfection was the dangerous case. Among 38 patients infected with both ALTNV and DBV, respiratory symptoms (61% vs. 38%) and kidney impairment (84% vs. 66%) were more common than in DBV infection alone, and seven of the 38 — 18.4% — died.

DBV disease is found throughout Asia, carries a case-fatality rate of 10% to 30%, and is listed by the World Health Organization as a research priority. Laboratory experiments showed the Asian longhorned tick can transmit ALTNV to mice.
